Challenging case involving a patient with monoparesis of the right lower limb. After a thorough cranial CT-CT angiographic examination, occlusion was confirmed in the basilar and bilateral cerebral artery posterior. Immediate systemic thrombolysis was initiated.

Dr. Tamás NÉMETH, neurointerventionalist and neurosurgeon from University of Szeged, Department of Neurosurgery, led the intervention. Utilizing a right distal radial artery approach, Dr. NÉMETH performed a meticulous procedure where the thrombus partially dissolved and drifted into the P2-P3 transition zone of the ACP on both sides. With precision, he successfully opened the left ACP using a Penumbra RED62 aspiration catheter, followed by the right ACP.

The thrombectomy concluded with a successful recanalization (eTICI3 result), fully opening both vessels and the basilar artery along with its entire branch system.
